I am writing to inform you that Fr. ___________________ is an incardinated priest in good standing in the Arch/Diocese of ______________________. He is currently serving as (title) for (parish/apostolate/diocesan office, etc.). This letter is for the purposes of the National Leadership Forum on Ministry with Young Adults to be held Nov. 30 to Dec. 1, 2016 in San Jose, California.  

We have carefully reviewed our records and have consulted with persons who have served with Fr. _______________ and I am able to make the following statements in regard to him: He is a person of good moral character and reputation. I know of nothing which would in any way limit or disqualify him from this ministry. He has completed safe environment training for our diocese on ___________.
