Welcome to the Diocesan Director’s Grant Evaluation page! 

If grant applications were submitted from your diocese, you will receive an e-mail inviting you to evaluate those applications.  

Click on the "Online Evaluation Login" link to log into the Reviewer Portal, CCHD's online application review system, to access the applications and submit your review.

Please familiarize yourself with the materials found below, the materials contain important updated information regarding the CCHD evaluation process.

*Important Note* - Word versions of the evaluation forms are for use with local committees who assist with the evaluation of grant applications. The final, completed evaluation form must be submitted through the Reviewer Portal, CCHD's online grant application review system.
